Food and Nutrition Services Nondiscrimination Statement:

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: https://www.usda.gov/sites/default/files/documents/ad-3027.pdf, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:

Request for Special Dietary Accommodations
Special Dietary Needs
If a student has special nutritional needs a Special Dietary Prescription for Meals at School Form will be given to the parent. This form needs to be signed by a Washington State authorized medical authority within 60 days. Exception is made in the case of milk substitution for students without disabilities. In that case, a parent may sign. A copy of the Milk Substitute Notification Form will be submitted to OSPI (copy included)
Meal modification will be provided to the student as soon as the school is aware of the need.
All forms will be given to the school nurse who will notify all staff of student dietary needs and a copy will be submitted to OSPI for approval.
Documentation is kept in the student’s health records. A copy is also kept in a dedicated food service file.
